I’m coming in for a Gateway Assessment.
What should I expect?
Before the Gateway Assessment
The Gateway Team will have received information about your health from any health services you may have seen llike your doctor GP, plunket nurse, or the hospital.
The Gateway Team will ask for information about your education from your school.
If your caregiver/parent gave permission for their medical records to be checked – the Gateway Team will check to see if you have any family health issues which could be impacting you in some way.
At the Gateway Assessment
You will come to the Gateway clinic to see some of the Gateway team probably a doctor or nurse.
You will have time to tell the doctor or nurse what you think about your health and education. The doctor or nurse will listen to what you say. What you say is very important to them. The doctor or nurse will check your health. This may include things like a hearing and eye test, listening to your heart and lungs, looking at your skin, and offering immunisations if you need them.
After the Gateway Assessment
The Gateway team will write a report. They will make suggestions about things that might help your health and support with education if needed. Your social worker will talk to you about it, and let you know if the team made reccomendations or referrals.
Gateway
You are coming to meet a doctor or nurse who will check you are well and healthy. Sometimes you go to the doctor when you are sick. Sometimes you go to the doctor when you are hurt. Sometimes you go to the doctor to check that you are growing big and strong.
The doctor or nurse will check to see how tall you are, they will look in your ears and listen to your heart beating and your breathing. They will ask you questions – you can ask them questions too!
There is a playroom and snacks if you need a break or get hungry.
If you need any medicine or treatment they will tell you, you can ask questions too.
When we are finished you will go back home and they will write a report (story) about meeting you and all the things they noticed. That report will make it easier for the adults that take care of you to make choices about your healthcare. You are the most important person, and what you think matters.
